Nu-Life helps customers request a review of insurance eligibility and the product-specific HCPCS E0603 breast-pump benefit. Active insurance does not by itself prove that a selected product is covered. Coverage, medical-necessity, timing, prescription, network, authorization, and payment rules depend on the individual health plan.
You agree to provide information that is accurate to the best of your knowledge. Your intake signature authorizes Nu-Life to use the submitted information for the displayed review and to contact your insurer or healthcare provider for that purpose. It does not place an order, authorize a card charge, guarantee payment, or replace a prescription.
No product, patient price, insurance amount, order, shipment, payment, or claim is final until Nu-Life completes its review and provides the applicable written decision or quote. If payment is required, double-check the product, amount, customer information, shipping details, and coverage decision before paying. Medical and hygiene products may not be returnable or refundable after processing, opening, use, or shipment, except where required by law or the written policy provided with the final quote.
You consent to electronic records, communications, and signatures for the intake request. A separate acceptance and signature will be required for any future final quote or payment authorization.
