When you request a breast-pump coverage review, Nu-Life may collect your contact and shipping details, health-plan and member information, the product you are considering, provider contact information, due or birth date, prior covered-pump information, acknowledgements, and your electronic signature.
The Phase 1 intake does not accept prescription or insurance-card uploads and does not collect card information. If payment is later required, Nu-Life will provide a separate written quote and approved payment path.
We use the information to review identity, active insurance, the product-specific HCPCS E0603 breast-pump benefit, prescription requirements, and available equipment options; to contact your insurer or healthcare provider when authorized; and to communicate the result of our review.
An eligibility response is not a guarantee of coverage or payment. No product, patient price, order, shipment, charge, or claim is final until Nu-Life completes its review and provides the applicable written decision or quote.
The secure intake uses HTTPS in transit, application encryption for the case record, encrypted signature storage outside the public web directory, restricted staff access, and audit records. Form contents are not placed in website analytics, ordinary email, public URLs, WooCommerce orders, or Square payment records during this intake phase.
We may share the minimum necessary information with health plans, healthcare providers, and service providers that support the requested review, or when required or permitted by law. We do not sell your health information.
Unsubmitted form entries stay in the active browser session. A submitted intake that is not used for a formal review is scheduled for secure deletion no later than 90 days after inactivity. Signed or reviewed case records are retained for at least six years. Fulfilled, billed, denied, appealed, audited, or recoupment-related records are retained for ten years or longer when a payer contract, law, accreditation rule, minor-record rule, or legal hold requires it.
